Required Documentation for New Players
- Birth Certificate Required
- Form 141-PCAHA Player Registration Form Required
- Two Proofs of Address Required (see options below)
• Utility bills (hydro, gas, water) – preferred (we require the complete first page, including pay stub) • Cable/satellite • Property/Contents insurance • Property tax bill/Assessment • Residential lease (must be complete and signed) • Bank/Visa statement • BC medical statement • GST statement, income tax assessment, child tax credit • School registration confirming address • Car insurance
